GENERAL CONCENT FORM 2021

Dear Families of Newtown PS,

Thank you to those who have already returned their child’s General Consent Form.

If you have not yet returned you child's General Consent Form for 2021, please do so as a priority.

Please assist us to support your child's learning by returning this annual permission form tomorrow.

Children without permissions will not be able to participate in local walking trips with their class and other aspects of learning may be impacted due to lack of permission from parents.

Please ensure you follow the instructions and initial each section to indicate your understanding of the permission you are giving in each instance before signing off at the bottom of the form.
